Necessary documents:

  • Request for payment of compensation;
  • Document about an event that has occurred;
  • Medical documents /invoice - original, epicrisis, outpatient lists, etc./;
  • Death certificate, Certificate of heirs, Bank account of the heirs (in case of accidental death);
  • Certificate from TELK / NELK (in case of permanent loss of working capacity as a result of an accident);
  • According to each specific case and event, the insurer may require additional documents

Important!


The insurer has the right, within 45 days from the date of filing the claim, to request additional documents and information, according to Art. 106 and Art. 107 of the CC for determining the basis for payment and the amount of insurance compensation.
