Accident and General Illness Claim

In case of permanent loss of working capacity as a result of an accident:

Necessary documents:

  • Request for payment of compensation;
  • Document about an event that has occurred;
  • Medical documents /epicrisis, outpatient lists, etc./;
  • Certificate from TELK / NELK;
  • Bank account

In case of accidental death:

Necessary documents:

  • Request for payment of compensation;
  • Document about an event that has occurred;
  • Medical documents / epicrisis, outpatient lists, etc./;
  • Death certificate;
  • Certificate of inheritance;
  • Bank account of the heirs

In all other cases:

Necessary documents

  • Request for payment of compensation;
  • Document about an event that has occurred;
  • Sick leave;
  • Medical documents / epicrisis, outpatient lists, etc./;
  • Original payment documents;
  • Bank account

The insurer has the right, within 45 days from the date of filing the claim, to request additional documents and information, according to Art. 106 and Art. 107 of the CC for determining the basis for payment and the amount of insurance compensation.
